My ex-husband and I have been divorced for five years. He hasn't seen our daughter in six years(by his own doing). In the divorce decree, I was given custody of her. The divorce decree doesn't state legal or physical custody, it states "Ms. Angelinmd is awarded custody of the minor child". He didn't show up for the hearing. He hasn't called his daughter in three years until today. We will be moving to Georgia in May. I notified the only person in his family that keeps in touch, his grandma. She must have contacted him because I received a message on the phone today from him. Do I have to have his permission to move? He wasn't given any visitation in the divorce decree because he didn't come to court or request any. My ex-husband hasn't paid any child support in six years and we don't know his whereabouts. What are my options?

I would go to court and get the move approved. It could cause you future grief if you don't. He won't show up anyway, so your attorney fees won't be much.
