I can only answer a couple of your questions, the one's that pertained to my divorce in NE a year and a half ago. I did have a lawyer but it was through a program at work so I can't tell you exactly what it cost. My lawyer typed up what we thought was fair and sent it to the ex's lawyer. They made a couple changed and in the end we agreed so I do believe it's best to agree before going to a lawyer. It would have been a little quicker.
Once we had the paperwork we took it to the judge. Since paperwork was already signed the ex didn't even come. I had to get up on the stand and basically my lawyer just asked me yes/no questions verifying who we were, child's name, date of birth, that type of thing. The whole court appearance lasted maybe 10 minutes.
The best way to do it is if you can agree before going to the judge. It saves money and time.
I'm not positive but I believe you'd have to live in another state for 6 months before you could file there. That maybe a state to state thing though.
